Job overview

This is an exciting time to join the Care of the Older Person and Frailty teams at Blackpool Victoria Hospital. We are developing our pathways to improve the experience our patients receive. This includes reducing length of stay and ensuring patients are discharged back into the community and supported by our Community Frailty Team.

The post holder will be a key member of the multi-disciplinary team to provide clinical quality care to all patients within the Care of the Older Person wards and the new Frailty Assessment Area (FAA) at Victoria Hospital. The role will also include development of the team and expanding the clinical role. You will be expected to provide expertise and decision-making skills to inform clinical reasoning when dealing with individual patient presentations.

As a Trust, we recommend that individuals employed in patient facing roles, or those located on a ward or department where patients receive a service are fully vaccinated against COVID-19.

Be proactive, demonstrating increased autonomy, clinical judgement and decision-making skills.

Act as an expert practitioner demonstrating advanced skills and knowledge in medical conditions, rehabilitation and complex patient needs.

Demonstrate expert knowledge in physiological and psychological functioning when there are diseases and disorders and producing an effective plan of care.
